Medical Records The following medical records must accompany the patient for the initial consultation:
- Medical history
- Chest x-ray report, if done
- Most recent CT and/or MRI scan films of the tumor area
- The radiologists's report about the CT/MRI scan results
- Pathology/Surgical reports
- CBC and chemistry battery report
- If previous radiation has been given, bring copies of treatment records, isodose distribution, and port films (actual x-ray films)
